It is important to the Evolve organization that we ensure our clients health needs are met on all levels. The purpose of case management is to provide our clients with specific resources to take control of his/her life.

Evolve Case Management services are designed to empower the client to make educated decisions about their future, we offer:

• Assistance in accessing services
• Provide ongoing service coordination
• Monitoring and support for services implemented

Our goal is to ensure clients are receiving the most appropriate and effective services available to meet their individual needs. To accomplish this, our counselors develop a plan in collaboration with their clients which reflect their choices and preferences for the service arrangements in a dynamic way.
